INSURANCE PLANS ACCEPTED AT OUR OFFICE

If your plan is a PPO, it will most likely be accepted.
If yours is a DHMO, or DMO, you are typically assigned to a specific dental provider that will be printed on your insurance card.  We are currently not accepting DMO plans.  
To be sure, please provide our office with your insurance information.
